CUSTODY SERVICES BUREAU PUBLIC REQUEST/COMPLAINT PROCEDURE: 

  1. This Americans with Disabilities (ADA) request/complaint procedure for public areas is established to meet the requirements of the ADA. It may be used by anyone who wishes to file a request/complaint based on a disability in the provision of services, activities, programs, or benefits by the Contra Costa County Office of the Sheriff’s Custody Services Bureau.

  2. The request/complaint should be in writing and contain information about the request/complaint, such as name, address, phone number of requestor/complainant and the location, date, and description of the issue. Alternative means of filing a request/complaint, such as personal interviews or a tape recording of the complaint, will be made available to accommodate persons with disabilities upon request.

  3. The request/complaint should be submitted by the requestor/complainant or his/her designee within 14 days to:

  4. The ADA Coordinator will investigate the matter and, within 45 calendar days after receipt of the request/complaint, will respond in writing and, where appropriate, in a format accessible to the requestor/complainant, such as large print, braille, or audio tape. The response will explain the position of the Contra Costa County Office of the Sheriff and offer options, if any, for a substantive resolution of the request/complaint.

  5. If the requestor/complainant does not believe the response by the ADA Coordinator satisfactorily resolves the issue, the requestor/complainant or his/her designee may appeal the decision to the Custody Division Commander.

  6. The Custody Division Commander will conduct an investigation into the appeal and, within 45 calendar days after receipt of the appeal, respond in writing and, where appropriate, in a format accessible to the requestor/complainant, with a final resolution of the request/complaint.

  7. Requests/complaints by in-custody individuals should be submitted using the ADA Inmate Grievance form or an Inmate Request Slip which is located on every housing unit at every facility.
